We operate a structured referral and assessment pathway to ensure service suitability, safety, and clarity before support begins. This ensures each placement is appropriate and properly planned.

We accept referrals from:

  • Local authorities

  • Social workers

  • Leaving care teams

  • Adult social care teams

  • Housing or commissioning teams

Information required when making a referral

For a quick and appropriate response to your referral, please include critical information such as:

  • a risk assessment

  • support needs summary

  • placement objectives

  • funding confirmation.

Referrals may be received from local authorities, case managers, health professionals, families, or self-referrals. We gather initial information regarding needs, risks, goals, and support requirements.

We review within 24–48 hours

Submit referral information by email

We review available assessments, care plans, risk records, and professional reports. Where necessary, we request further documentation to ensure informed decision-making.

Risk and compatibility assessment completed

Submit referral information by email

We conduct an assessment meeting with the individual and relevant stakeholders. This may include family members, social workers, therapists, or advocates. We evaluate needs, compatibility, risks, and outcome goals.

A formal suitability decision is made based on needs, risks, staffing capability, and environment fit. Where we cannot safely meet needs, we are transparent and decline the placement.

Placement decision confirmed

If accepted, we develop a detailed support plan and transition schedule. This includes risk controls, staffing levels, routines, and outcome targets. Transition is paced and structured to reduce anxiety and risk.
