Adults with Physical or Sensory Disabilities

We support adults with learning disabilities, physical disabilities, and sensory impairments through tailored support arrangements that promote independence, dignity, and inclusion. Each person receives an individual support plan based on a detailed assessment of needs, strengths, and communication preferences. Support may include daily living skills, mobility assistance, personal care, communication support, and access to community activities. We make reasonable adjustments and use adaptive approaches to ensure accessibility and equal participation.

Adults with Mental Health Needs

We support adults experiencing mental health needs using recovery-focused and strengths-based approaches. Our support promotes emotional stability, personal choice, and self-management skills while maintaining robust risk and safeguarding frameworks. Plans include wellbeing goals, structured routines, relapse awareness, and social inclusion. We coordinate with clinical teams and community services where appropriate to maintain continuity of care and safe outcomes.

Young persons 16+

Our services are specifically designed for teenagers aged 16 and above, providing them with the guidance and support they need during this pivotal stage of life. We focus on fostering personal growth and resilience to help them.

Older People

We support older adults who wish to remain as independent as possible while receiving flexible and responsive assistance. Support focuses on maintaining daily functioning, safety, and quality of life. This may include help with routines, mobility, nutrition, medication prompts, and social engagement. Care is delivered with dignity and respect, with regular review to reflect changing needs and preferences.

Our Transition & Specialist Pathways service provides essential support for individuals transitioning from rehabilitation and residential care to independent living. We specialize in hospital discharge support, brain injury pathways, preparing for adulthood, ensuring a smooth step-down process for each individual

Adults with Mental Health Needs

We support individuals transitioning from hospital or rehabilitation settings following acquired or traumatic brain injury. Our approach is rehabilitation-informed and outcome-driven, focusing on rebuilding practical skills, cognitive coping strategies, and structured independence. Support plans align with therapy recommendations where available, and progress is reviewed against measurable goals to ensure safe and sustainable development.
